WHAT to do if you are in need of normal medical treatment?

Out-patient treatment :
Just visit a legally registered medical practitioner of your choice. Be sure to take the Medical Claim Form with you. Ask the doctor to complete, stamp and sign the Medical Claim Form.

If the out-patient treatment results in an in-patient treatment, please see the following.

In-patient treatment :
Prior to in-patient treatment you must contact AWC (Allianz Worldwide Care) to obtain a Treatment Guarantee/Pre-authorization.

A Treatment Guarantee/Pre-authorization is also required for several other benefits mentioned in the Allianz Healthcare Plan. Please refer to section 2 of the notes on the Table of Benefits.

WHO pays the doctor or the hospital?
You are a self-paying patient. The doctor is expecting your payment. Please pay the bill on time. Send the original invoices, medical treatments costs etc. to  AWC (Allianz Worldwide Care). Note that medicine and drugs are only covered if prescribed by a registered doctor. Please make sure that the medical claim form is duly completed and signed. The reimbursement (less the deductible) will be sent to you by transfer of funds or by cheque.

WHAT can you do if a crew member falls seriously ill?

For emergency cases please call the Allianz Worldwide Care 24-Hour Hotline. This hotline is able to confirm treatment guarantees in case the crew member has to go to hospital. The invoices will be sent to AWC (Allianz Worldwide Care). Please read more about this under section 2 of the notes on the table of benefits.
